Your membership allows you to receive the most Affordable and Effective Integrative and Naturopathic Medical and Wellness Care in AZ! Non members are certainly welcome and receive care through our sister company Westhampton Natural Medicine at the same location, but an AZP membership will save you a great deal of money while we assist you to prevent disease, treat illness and improve health.

AZP FAMILY HEALTH PLAN: (This includes mom, dad and children living at the same residential address.)

• Monthly membership fee is $25.00 per family (minimum one year agreement)

• OFFICE VISIT #1: $150.00 per person.
  This one time fee establishes you as a patient and includes a comprehensive medical history, physical examination, health risk assessment and diagnosis.

• Laboratory diagnostic tests requiring physical samples shall incur additional fees.

• OFFICE VISIT #2: $75.00 per person.
  This includes a comprehensive review of laboratory test results and a personalized medical treatment and wellness program.

• ALL OTHER OFFICE VISITS: $25.00 per person regardless of complexity or duration.

• All treatments, therapies and prescription items are provided at a 25% discount.

• We like to keep things simple, so monthly membership fees are collected via Electronic Funds Transfer (EFT). Simple and easy!

• This plan is ideal for families, individuals and business owners who want to keep medical and health expenses to a minimum while achieving health with or without medication.
