FAMILY MEMBERSHIP PLAN: (for families and individuals living at the same residential address)

• Monthly membership fee is $25.00 per individual/family, with one year agreement.

• Office visit #1: $125.00 per person. This one time enrollment fee establishes you as a patient and includes a comprehensive medical history & consultation, physical examination, health risk assessment and diagnosis.

• Office visit #2: $75.00 per person. This includes a comprehensive laboratory test results review and a personalized medical treatment and wellness program.

• Office visit #3 and beyond: $25.00 per person for all subsequent visits, regardless of complexity or duration.

BUSINESS MEMBERSHIP PLAN: (for business owners and their team doing business at a commercial address)

• Monthly membership fee is $25.00 per person, regardless of complexity or duration.

• Office visit #1: $75.00 per person. This one time enrollment fee establishes you as a patient and includes a comprehensive medical history and consultation, physical examination, health risk assessment and diagnosis.

• Office visit #2 and beyond: $25.00 per person for all subsequent visits, regardless of complexity or duration. If the second visit involves a comprehensive review of laboratory test results, the fee for that visit is $75.00.

• Business payment options: 1) You the employer could pay the monthly fee for your employees, which includes their family members who live at the same address. This option would make you very popular and help to improve employee loyalty, morale, and productivity! 2) Your employees could pay their own monthly membership fees which could by deducted from payroll. The quality of care and overall benefits received would be well worth it! 3) You could share the monthly fee with your employees at any percentage you feel is appropriate. For example, if a 50/50 shared split is chosen, business members would enjoy personalized, medical and wellness care for a mere $12.50  per month per family! That’s a pretty amazing deal!

• We like to keep things simple. So rather than collecting monthly fees from everyone in your company every month, we would keep your company credit card on file for your convenience and automatically charge the total monthly amount for all members via Electronic Funds Transfer (EFT). Simple and easy!
