1. INDIVIDUAL/FAMILY PLAN

• Monthly membership fee is $25.00 per individual/family.

• Office visit fee is only $25.00 per appointment.

• One time enrollment fee is $75.00 per person. This enrollment fee establishes you as a patient and includes a comprehensive medical history & consultation, physical examination, health risk assessment, diagnosis and a personalized medical and treatment program.

 2. BUSINESS PLAN

• Monthly membership fee is $25.00 per individual/family.

• Three primary payment options exist. 1) You the employer could pay the monthly fee for your employees, which includes their family members. This option would make you very popular and help to improve employee loyalty, morale and productivity! 2) Your employees could pay their own monthly membership fees which could be deducted from payroll. The quality of care and overall benefits received would be well worth it! 3) You could share the monthly fee with your employees at any percentage you feel is appropriate. For example, if a 50/50 shared split is chosen, business members would enjoy personalized, medical and wellness care for a mere $12.50 per month per family! That’s a pretty amazing deal! We like to keep things simple. So rather than collecting monthly fees from everyone in your company every month, we would keep your company credit card on file for your convenience and automatically charge the total monthly amount for all members via Electronic Funds Transfer, (EFT). Simple and easy!

•  One time enrollment fee is $25.00 per person. This enrollment fee establishes each person as a patient and includes a comprehensive medical history & consultation, physical examination, health risk assessment, diagnosis and a personalized medical and wellness treatment program.

• Office visit fees are only $10.00 per person for routine wellness and medical check ups.
